Chair’s Evaluation – A.S. Honoraria

To be eligible for honoraria, per Legal Code:

Appointed and Elected Officers shall be required to complete a minimum of six (6) units per quarter and maintain at least a two point zero (2.0) cumulative grade point average during the term of office.

Chair’s Evaluation Form

The Honoraria Committee asks that all chairs submit a formal evaluation of their members. Please provide relevant information regarding the completion of their listed duties, extraordinary accomplishments, and other achievements for all members of your board, commission, or unit. Refer to the A.S. Legal Code section pertaining to each position as an outline for your comments. Feel free to make additional copies as necessary.

All names will be kept confidential. Reminder that BCU & office members are eligible to receive max $300 each quarter. Deputy head/chiefs of staff are eligible to receive $450.
